Hampshire Partnership Board / PCP team can offer support & help with Person Centred Planning & approaches. Are you a person with a learning disability, family, residential providers, Support staff, managers, schools, transition, community services or any person that wants to help people with a learning disability plan for the future.
Hampshire PCP team can help you start your first plan or help providers develop person centred approaches.
